Advance Care Planning Assistance

FreeTransitions LifeCare and the NC Bar Association along with other national, state, and community organizations, are leading a massive effort to highlight the importance of advance care decision-making – an effort that has culminated in the formal designation of April 16 as National Healthcare Decisions Day (NHDD) and Health Care Decisions Day in North Carolina.
The North Carolina Bar Association, NC Partnership for Compassionate Care, local attorneys, and Transitions LifeCare are providing information and tools for the public to talk about their wishes with family, friends and health care providers, and to execute written advance directives (Health Care Power of Attorney and living will) in accordance with North Carolina state laws.
Specifically, on April 8, 2017, from 11:00 a.m. to 1:00 p.m., Transitions LifeCare is welcoming the public at their Raleigh campus located at 250 Hospice Circle (27607) with free information about advance care planning and assistance with completing Health Care Power of Attorney and living will forms.
